Diabetes and dementia: what blood sugar has to do with memory

I've had type 2 diabetes for ten years. Control isn't always perfect. Does diabetes really increase the risk of dementia – and does better control protect against it?

The association exists: type 2 diabetes is linked to an increased risk of cognitive decline and dementia. But the right message isn't one of fear – it's one of motivation: what you already do (or should do) for diabetes also protects your brain.

What the WHO says

Diabetes management should be offered to all adults with diabetes, according to general guidelines – this is indisputable. Specifically, to reduce the risk of cognitive decline and dementia, the guidelines add that diabetes management can be offered for this purpose (conditional recommendation; the certainty of specific evidence is still very low – meaning it's not yet proven that glycaemic control reduces dementia, but the rationale is plausible and control is already fully justified for everything else).

How diabetes affects the brain

Chronically high sugar damages blood vessels, including the very fine ones that supply the brain. Diabetes also increases the risk of stroke – and each cerebral vascular damage is one less brick in the brain's reserve. Controlling blood sugar, blood pressure, and cholesterol together is protecting this plumbing.

Ten years of chronic illness is tiring – lapses in discipline are human, not character flaws. Instead of pursuing perfection, aim for achievable consistency: medication on time (alarm, weekly pill box), regular appointments without missing them, and control tests up to date. If the current regimen is difficult to follow, tell your doctor – simplifying treatment is often possible and improves control more than willpower.

The boost that comes from outside of pills

Regular exercise and a balanced diet are serious treatments for type 2 diabetes – sometimes as effective as an additional medication. And these are exactly the same measures that the WHO recommends for the brain. In prevention, everything adds up.
